All applicant Caregivers are screened to determine qualifications and competence as described in the current standards of the State Dept of Health you are currently applying in. 

 

  • At least one (1) year of experience as a Certified CAREGIVER

  • Proof of compliance with applicable immigration laws and maintenance of I-9 documentation..

  • Current health history, including a record of immunizations (MMR & Varicella), current PPD skin test or Chest X-Ray / TB Mantoux Test , and Physical Examination.

  • A minimum of one (1) year professional references of past or current employment. 

  • Successful completion of pre-employment drug screening. Recent 9 panel drug screen.

  • Clearance of appropriate criminal background checks.

  • Original and current driver license, US Passport or State ID.

  • Original Social Security card.

  • IDPH Certification (for IL Caregivers) FL-AHCA Background Screening (BGS for FL Caregivers) 

  • Original and current American Heart Association CPR/BLS for the healthcare provider.

  • Completed physical examination within the year including Snellen (Eye Exam).

  • TB skin test (PPD) within the year. If the results are positive, we require a CXR within 2 years. .

  • Immunization records for MMR, Varicella, DTAP, Hepatitis B and Hepatitis C.

  • Certificates of continuing education.
